Make An Appointment
There are several ways to make an appointment at our practice. You may call us at 423-626-7070, e-mail us through our website, or drop by the office in person. We welcome new patients and treat children whenever possible.
Our appointments are extremely important to us, and we work hard to stay on schedule. In order to accomplish this, we inform you often of your appointments, we ask you to arrive a few minutes early, and we request that you notify us of any necessary changes two working days before your appointment day.
Once you make your first appointment with us, we will mail you a New Patient Packet which will include our registration form. In the case that we do not have ample prior notice to send you this packet, we will encourage you to come to this website site, open this registration form, print it, complete it, and bring it in with you to your appointment.
Either way, having the form prior to coming to your appointment will allow you to complete it at your convenience and in the comfort of your home where you can easily obtain phone numbers, names of your medications, and insurance information. If you have questions concerning this registration form, please call our office at 423-626-7070.
